Your physician’s office may schedule a pre-admission appointment with us prior to your surgery. During this appointment you will need to provide personal, medical, and insurance information to our pre-admission staff.
You will also be given information concerning patient right’s, anesthesia risk, and other information relating to healthcare privacy.
The forms they will use are listed below and may be printed, filled out in advance, and brought to your appointment. This will expedite the pre-admission process.
The business day before your procedure you will receive a phone call from the surgery center to go over arrival time, reminders and to answer any last minute questions you may have. The day of surgery you will check in at the front desk where preparation for your procedure begins. The first business day following your procedure you will be contacted by one of the nurses to follow up and ensure you have no questions or issues.
